PSP Report Terms

Version 2026-10-14 · These terms apply to every FMCSA Pre-Employment Screening Program (PSP) report you request through FleetCollect, in addition to the FleetCollect Terms of Service.

1. What a PSP report is

A PSP report is the driver’s crash history from the last five years and roadside inspection history from the last three years, from the Federal Motor Carrier Safety Administration’s (FMCSA) Motor Carrier Management Information System. FleetCollect (Solve For Solutions LLC) requests the report from the FMCSA PSP system on your behalf and makes it available to you. FleetCollect does not create, change or verify the data.

2. Permissible purpose — you certify

Each time you request a report, you certify that:

  • you are requesting it only to evaluate the named driver for employment (or a contracted driving position) with the company named as “Prospective Employer” on the driver’s authorization, and for no other purpose;
  • the company named as Prospective Employer is your company, and you are authorized to act for it;
  • you will not resell, share or disclose the report except to the driver or as required by law.

3. Driver authorization

FMCSA requires the driver’s signed authorization, on the exact FMCSA PSP Disclosure and Authorization form, before any report is requested. FleetCollect collects it electronically and will not request a report without it. The authorization must be signed on or before the request date, with the driver’s full legal name. If you collect the authorization yourself, for example by uploading a signed form, you are responsible for making sure it is the unaltered FMCSA form, signed by the driver, and not combined with any other document.

4. Your responsibilities if you act on a report

The authorization the driver signs describes the Fair Credit Reporting Act (FCRA) steps a Prospective Employer must follow before and after taking adverse action, such as declining to hire, based on a PSP report. Those steps include giving the driver a copy of the report and a summary of their rights, and the required notices. They are your responsibility as the employer. FleetCollect does not make or advise on hiring decisions.

5. Accuracy and disputes

Neither FleetCollect nor FMCSA’s contractor can correct PSP data. Drivers can challenge it through FMCSA’s DataQs system at dataqs.fmcsa.dot.gov. A report lists every reported crash regardless of fault, and every inspection with or without violations.

6. Pricing and refunds

  • FleetCollect DQF customers: $19.00 per report, charged to your card on file when FMCSA returns a result. That includes a “no record matched” result, because FMCSA charges for it too. You are not charged if FMCSA rejects the request because of invalid data, or if the request doesn’t go through.
  • Self-serve orders: $19.00 per report, paid when you order. You can cancel for a full refund until the driver starts the authorization. After the driver signs, the report is requested and the fee is non-refundable, including when no record matches.

7. Records we keep

FleetCollect keeps the driver’s signed authorization, the request details and the report with your account for at least three years, as the PSP program requires, even if the driver is not hired. Driver license details and date of birth are stored encrypted. You should also keep your own copies for your records.

8. Program rules

PSP access is provided under FMCSA’s program rules, and the program audits compliance. If a request or its authorization does not meet those rules, FleetCollect may decline or cancel it, and may suspend PSP access for your account.
